Did you know that you have the right to give instruction about your own health care?  You also have the right to name someone to make health care decisions for you.  These online advance directives  will  let you do both.  Write down your wishes and the selection of your primary doctor. Georgia

Two  witnesses have to sign a  GA Advance Directive for it to be considered valid. Witnesses must be:

                   At least 18 years of age, un- related to the person whose advance directive is being created or  a person who will inherit property or money from the person in the advance directive or  responsible for paying the medical bills of the person whose Living Will is being created . The first and second witness must be someone other than, a doctor  or an employee of the hospital or nursing home .

 If the advance directive  is created while the patient is in a hospital or skilled care nursing home, he/or she MUST have an additional person sign the form. This third witness has to be the medical director of the skilled nursing home OR staff physician, someone else  besides those participating in the care of the patient. If in a hospital, it has to be the chief of the hospital staff or staff physician.

South Carolina*
